  • Major Duties andResponsibilities
  • Participates in planning and implementing ofyour unit’s activities on the shift and unit as assigned by the DONand/or RN Supervisor.
  • Assure that department personnel, residents,visitors, etc. follow established policies and procedures, toinclude safety rules and regulations, by interceding when aviolation occurs.
  • Provides accurate and consistent information topersonnel, residents, visitors, family members, etc. as necessary.
  • Ensure that resident’s rights to fair and equitable treatment,self-determination, individuality, privacy, property, and civilrights, including the right to a complaint, are followed by alldepartment personnel as given in the facility’s establishedpolicy.

  • Assure that astock level of medications, medical supplies, equipment, etc. aremaintained at adequate levels. Requisition equipment and supplyneeds as required.
  • Notify the resident’s attending/alternatephysician, RN supervisor, as well as the resident’s legal guardianrepresentative, when the resident becomes critically ill, injured,etc. as outlined within our established policies.
  • Work with thefacility ‘s consultant s as necessary and implement recommendedchanges as directed by the DON.
  • In cases of death, may call thefuneral home as requested by the family, or as otherwise outlinedin our established policies. Assure that the body is properlyprepared/cleaned for pick up. May announce death as allowed withinthis state’s rules.
  • Make oral reports/recommendations forinterventions for nursing POC to the supervising RN/DON concerningthe delivery of resident care on the nursing unit/hall assigned.
  • Assure that the unit area is maintained in a clean and safe mannerfor resident comfort and convenience by assuring that necessaryequipment and supplies are maintained to perform suchduties/services.
  • Notify physician and RN supervisor of residentaccidents/ incidents. Fill out and complete accident/incidentreport forms on all such occurrences and chart such information inthe resident’s medical record as outlined in our establishedpolicies and procedures.
  • Take TPRs, blood pressures, etc. andchart as required.
  • Gives/receives nursing report upon reporting inand ending shift duty hours.
  • To monitor residents throughout theassigned shift to assure that prescribed treatment is beingproperly administered as assigned to other LPNs or delegated tonursing assistants. Record findings in the resident’s chart
  • Communicate changes in resident condition to RN as needed accordingto agency policy.
  • Prepare/administer medications as ordered by thephysician and according to agency policy (
NOTE:

Another nurse MAYNOT administer medication set up by another nurse)

  • 1Chart nurses’notes in an informative and descriptive manner that reflectspertinent information and the care provided as well as the resident’s response to the care.
  • Complete required record keepingforms/charts upon the resident’s admission, discharge, transfer,etc. File in the resident’s chart and/or forward to appropriatedepartment.
  • Receive verbal orders from attending/alternatephysician and chart on the physician’s order sheet and inform RNsupervisor of changes per this facility’s established policies.
  • Assist in orientation of newly assigned unlicensed personnel tofacility-specific nursing procedures assigned by the RN as follows:
  • Demonstrating a specific task or technique according to thefacility’s established procedures
  • Observing an unlicensedpersonnel return demonstration of a specific task or technique incomparison to the facility’s established step-by-step procedure;and
  • Providing data regarding the unlicensed personnel performanceof the nursing task or technique to the registered nurse todetermine competency.
  • Submit an order for prescribed medications,supplies, and equipment as established by our policies andprocedures.
  • Review physicians orders, transfer/pre-admissioninformation, etc. for completeness
  • Orient new residents/familymembers as necessary according to policy
  • Forward new diet ordersand/or diet changes to the dietary department
  • Transcribephysician’s orders to resident charts, treatment plans, etc. toassure that they remain current.
  • Verify the identification of theresident before administering medication, treatment, etc.
  • Requisition and arrange either for diagnostic and therapeuticservices, as ordered by the physician, within the facility or byoutside arrangements in accordance with established policies andprocedures.
  • Assure that narcotic records are current.
  • Admit,transfer, and discharge residents according to policy. Assist inarranging transportation, packing, etc. as necessary.
  • Checkcurrent charts for specific treatment, medication orders,schedules, etc.
  • Administer nursing interventions, such ascatheterization; tube feedings; suction; applying and changingdressings/bandages, packs, colostomy and drainage bags; takingblood, sputum, urine specimens; massage and exercises; care of thedead/dying; etc. as established by our policies and procedures andnursing plans of care.
  • Assure that personnel follow establishedinfection control and aseptic techniques when isolation proceduresbecome necessary.
  • Make rounds with the physician and assist withexaminations and treatment.
  • Notify physician of auto-stop orderson specific medications as established by our policies andprocedures.
  • Review Medication Administration Record forcompleteness of information, accuracy in the transcription ofphysician orders, and adherence to stop-order policies.
  • Carry outrestorative and rehabilitative programs, to include self-help/care,as per established plan of care.
  • May per form minor housekeepingduties around the nursing unit.
  • Perform routine charting asrequired.

  • Reports noncompliance with policies, procedures,regulations, or breaches in confidentiality to appropriatepersonnel. Reports any retaliation or discrimination to HR orcompliance officer.
  • Reports any allegations of abuse, neglect,misappropriation of property, exploitation, or mistreatment ofresidents to supervisor and/or administrator. Protects residentsfrom abuse and cooperates with all investigations.
  • Reports anyoccupational exposures to blood, body fluids, infectious materials,and/or hazardous chemicals in accordance with facility policy.
  • Participates in all life safety and emergency drills and trainings.

Fulfills responsibilities as assigned during implementation oractivation of the facilitys emergency plan.
