HOSPICE LICENSED PRACTICAL NURSE

That is why Liberty Hospice provides our hospice patients withstate-of-the-art care and pain management services, delivered byour specially trained staff with emphasis on strength, dignity andcompassion. We are currently seeking an experienced:

HOSPICELICENSED PRACTICAL NURSE

(LPN) Full-Time, Monday -Friday withon-call responsibilities (Covering Raeford and surrounding areas)

Job Description:

• Provides direct patient care, to include using avariety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy andprocedure and regulation.
• Follows the plan of care appropriate tothe patient’s diagnosis and condition as developed and assigned bythe Registered Nurse.
• Coordinates Hospice services with theRegistered Nurse, Patient Care Coordinator and other agencypersonnel to optimize patient care, as well as, the patient,family, community agencies, and physicians.
• Accepts andtranscribes physician’s orders as directed.
• Forwards allclinical, progress, and summary notes to the Hospice RegisteredNurse for coordination of care.
• Attends and participates in IDTmeetings.

Job Requirements:

• Successful completion of nursingeducation from an approved school of nursing.
• Current LPNlicensure in the state in which the branch is located and one yearof clinical nursing experience, preferably in an acute caremedical/surgical setting.
• Ability to independently organize workand provide effective health care services to hospice patients.
• Ability to ensure appropriate patient care as supervised by theHospice Registered Nurse, and document thoroughly, accurately, andcompletely on a timely basis.
• Effective communication skills(oral and written).
• CPR certification.
