None Summary of Position:
Performs treatments, procedures, and patient care in the clinic. Reports to the Team Clinic RN under the direction of the Clinical Services Director.
Education:
High School Diploma or GED Experience:
1-2 years as a LPN At least 1 years experience working with the frail elderly population.
Licensure, Registry or Certification Required:
Valid NC license, registry, or certification in the field of practice. Maintenance of current unrestricted driver’s license. Phlebotomy certificate.
Special Training:
Able to communicate effectively both verbally and in written form; able to lift and turn participants; knowledge of and care to adult/geriatric’s with chronic health conditions. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by Senior TLC. and approved by CMS before working independently.
Immunizations:
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact
Skills/Abilities:
Must be capable of performing job responsibilities. Ability to work effectively in a team environment. Excellent verbal and written communication skills. Ability to write and present information clearly. Able to work with adult geriatric patients. Proficient computer skills with MS Office products, knowledge of excel and previous EMR experience. Excellent customer service and communication skills. Ability to effectively handle difficult and unusual interpersonal situations. Consistently demonstrates responsibility, accountability and dependability. Key Responsibilities Fulfills participant care responsibilities as assigned by the Clinic Nurse or designee within the LPN scope of practice. Responsibilities may include but are not limited to: performs procedures and treatments under the supervision of the Clinic nurse or designee, which may include venipuncture, EKG, wound care (within scope of practice), urine specimens, etc. Provides personal care to participants at Senior TLC (bathing, dressing, feeding, toileting, transfers, lifting, and incontinent care.) Schedules participants to be seen in the clinic per IDT and assists with the participants navigating care throughout the encounter process to maximize participant satisfaction. Works with Clinic Nurse and Primary Care Provider along with the IDT to assist participants with holistic needs in the clinic setting. Documents participant care delivered as planned, variations & records observations in participant’s electronic medical record. Reports & documents in the EMR any changes in participant condition to team members. Assists nurse with obtaining and documenting vitals and completing basic triage care. Assists with transporting participants to the clinic. Assists with basic finger sticks and other labs. Answers phone calls, documents messages for clinic staff and assures closure of loop on communication with participants and caregivers. Makes & documents follow-up phone calls and communication to families and caregivers as directed. Supports Senior TLC’s mission by enhancing mobility, independence, quality of life, and quality of end of life for the participants Supports the Senior TLC IDTs and promotes unity amongst team members while interacting with the team, other co-workers, and/or participants. Supports team members and functions as a team player while providing that support and adheres to the “do no harm” statement. Accepts teammate’s professional and cultural differences as they relate to participant care and supports team’s decisions while promoting a united team for the participants’ continuity of care and for staff development. Follows up on Specialist visits as directed. Assists with Podiatry and Dental clinic as assigned. Cleans exam rooms in the clinic (i.e. make beds, change linens). Re-stocks clinic supplies. May assist with ordering supplies as needed. Maintains a clean/orderly workspace in the clinic environment. Properly disposes of bio-hazard waste. Participates in home services on call and/or weekend rotation for PCS Participates in staff meetings as required. Assists nurse/provider with any activities requested in the clinic, performs annual competencies and yearly evaluation within required time frames, and other duties as assigned.
