Summary:
Provides direct patient care,meeting both psychological and physical needs in accordance withthe plan of care, physician’s orders, and nursing and departmentpolicies and procedures. Patients range in age from neonate togeriatric. Care may be provided in an inpatient setting, ambulatorycare, home health, or hospice.
Responsibilities:
1. Contributes tothe development and maintenance of a current, individualized,goal-oriented plan of care and consults with other healthprofessionals to assure that the assessed needs ofpatient/caregiver are met.2. Observes, assesses, documents, andreports patient response to treatments/therapies and teaching, andadjusts priorities of care according to patient/caregiver needs.
Collects, documents, and reports (to responsible MD or RN) dataregarding patient’s biophysical, psychosocial, environmental,self-care, educational, and discharge planning needs.3.Participates in unit, service, and/or department enrichmentactivities which contribute to positive outcomes such as assignedcommittees, CQI activities/groups, and the orientation andeducation of other personnel.4. Provides a broad range oftreatments/therapies to patients including, but not limited to,respiratory treatments, Oral/NG/NT suctioning, and tracheostomycare, administration of medications, observation of cardiacmonitors, respirators, etc. and reporting of any changes,assistance to physician with special procedures, monitoring ofintravenous infusions, performs point of care testing, performsroutine laboratory work, changing/irrigating catheters, specializedsterile dressings, and patient/caregiver teaching based upon theestablished teaching plan.5. Provides assistance to patients andfamilies during admission to the clinical area, orientation to thehospital/clinic experience, and discharge.6. Provides support topatients and families. Verbally de-escalates frightened orpotentially aggressive patients. Facilitates age appropriate copingstrategies for patients.

Education Requirements
- Requires graduation from astate accredited program of practical nurseeducation.
Licensure/Certification Requirements
- Licensed topractice as a Practical Nurse (LPN) in the state of North Carolina.
Requires BLS.Professional Experience Requirements
- Requires two(2) years of previous experience as an LPN.
