We are currently seeking an experienced:
HOSPICE LICENSED PRACTICAL NURSE
(LPN) Full Time (Pitt County and surrounding counties)
Job Description:
- Provides direct patient care, to include using a variety ofskills and necessary coordination of services.
- Completes appropriate documentation as required by policy andprocedure and regulation.
- Follows the plan of care appropriate to the patient’s diagnosisand condition as developed and assigned by the RegisteredNurse.
- Coordinates Hospice services with the Registered Nurse, PatientCare Coordinator and other agency personnel to optimize patientcare, as well as, the patient, family, community agencies, andphysicians.
- Accepts and transcribes physician’s orders as directed.
- Forwards all clinical, progress, and summary notes to the HospiceRegistered Nurse for coordination of care.
- Attends and participates in IDT meetings.
Job Requirements:
- Successful completion of nursing education from an approvedschool of nursing.
- Current LPN licensure in the state in which the branch is locatedand one year of clinical nursing experience, preferably in an acutecare medical/surgical setting.
- Ability to independently organize work and provide effectivehealth care services to hospice patients.
- Ability to ensure appropriate patient care as supervised by theHospice Registered Nurse, and document thoroughly, accurately, andcompletely on a timely basis.
- Effective communication skills (oral and written).
- CPR certification.
