Full Job Description Schedule:
Availability to work a minimum of two 12-hour shifts every 4 weeks. Under the direct supervision of a Registered Nurse, the LPN (Licensed Practical Nurse) provides direct nursing care to patients in the IPU in accordance with the Nurse Practice Act and/or Hospice Standards of Practice. What are the essential duties day to day? Duties are delegated by and performed under the supervision of a registered nurse. Responsible for assessing patient/family needs and for implementing and evaluating the plan of care. Provides direct patient care according to the plan of care and in accordance with the Nurse Practice Act and Hospice Standards of Care. Provides nursing services (treatment of symptoms, preventative measures, etc.) according to the plan of care. Instructs family on IPU procedures as needed. Serve as an active member of the IDG and attend group meetings regularly. Continually re-assesses patient needs, notify physician, and under the direction of RN, modify plan of care as needed. Assists with chart audits to insure compliance with regulations. Actively participates in team and staff orientation. May pronounce death. Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals. Documents data collected for assessments and interventions in an accurate and timely manner according to the nursing standards of this agency. Ability to lift, turn and transfer patients; minimum lifting requirement 40 pounds. Here is what our employees have to say! We are a very close knit group, more like family!
