The LPN LTC is responsible for providing specialized care topatients in a long-term care setting. Working under generalsupervision, this role assesses, plans, and implementsinterventions to promote optimal healing and prevent complications.

This position collaborates with the interdisciplinary team toensure the delivery of high-quality care and adherence to bestpractices in wound management.

Talent Pool:

Nursing Conducts comprehensive assessments of patients’ needs to developindividualized care plans. Performs treatments following established protocols practices. Monitors and evaluates the progress of patients, documentingfindings accurately and communicating any changes or concerns tothe healthcare team. Educates patients, families, and caregivers on proper caretechniques, prevention strategies, and signs of complications. Collaborates with physicians, registered nurses, and otherhealthcare professionals to coordinate care, ensure continuity, andoptimize patient outcomes. Maintains accurate and up-to-date documentation of assessments,interventions, and outcomes in the electronic medical recordsystem. Performs other duties as reasonably required or assigned.

EDUCATION
Required:

High School Diplomaor equivalent; Diploma in Practical Nursing.

EXPERIENCE
Preferred:

1year acute careexperience
