Case Management Nurse Summary:

This is a part-time position (4 days/week, Fridays and weekends off) where the Nurse performs a combination of tasks, administrative duties, and direct patient care in a primary medical care office. Care Coordination includes continuity of health care services, bridging transitions, and addressing social issues that impact health outcomes. The Nurse is responsible for identifying high-risk patients, addressing quality issues, and providing patient education. We are looking for an energetic and welcoming Nurse to work alongside other health care professionals to deliver high quality medical care to uninsured adults of limited income and patients with Medicaid and Medicare. The MISSION of Grace Clinic is to glorify and serve God by providing free medical care, health education, and spiritual guidance to those in need. The Clinic also serves underinsured patients with Medicaid and Medicare. Usual hours are:

Monday 8:00-5:00; Tuesday 8:00-5:00; Wednesday 8:00-3:00; and Thursday 8:00-5:00. Since Grace Clinic is a small free clinic normally open four days per week, employees are part-time and employer-sponsored health insurance is not available. The position would receive paid holidays and PTO leave. This is a great position for an individual that would enjoy Friday’s off.

Responsibilities:

Provides “person-centered” care that is tailored to the individual. Encourages patients to follow the recommendations of Medical Providers. Creates and coordinates patient care plans. Identifies and addresses barriers to quality care and outcomes. Provides patient education for chronic diseases and acute medical concerns. Identifies high-risk patients and works directly with those patients to improve medical outcomes. Administers vaccines. Provides patient education. Coordinates special clinics such as eye screenings and women’s health. Participates in offsite outreach. Documents activity in Clinic’s electronic health record (AthenaHealth). Assists Intake Medical Assistants, as needed. Serves as a resource for volunteers and other clinical staff. Manages data in reports and spreadsheets. Grant reporting. Other duties as assigned.
