Workingclosely with a dedicated team of physicians and clinicians, you’lldevelop and manage care plans that support recovery and helppatients get back to the life they love.

As a Home Health LPN, YouWill Provide professional nursing services under the direction of aRN in compliance with the state’s Nurse Practice Act, anyapplicable licensure/certification requirement, applicablehealthcare standards, governmental laws and regulations, andCenterWell Home Health policies and procedures. Provide skilled nursing interventions in the treatment of thepatient/clients illness, rehabilitative needs and preventativecare. Utilize a holistic approach in the provision of problemspecific therapeutic interventions, teaching and trainingactivities in accordance with the plan of care. Apply knowledge and skills in accordance with acceptedstandards of clinical practice to facilitate problem resolution andachieve individualized patient goals and outcomes. Confer with the RN Supervisor regarding needed changes in thePlan of Treatment. Accept verbal orders from physician werepermitted by state law/regulations/Nurse Practice Act andcommunicate these orders to the RN Supervisor. Utilize a systematic, individualized goal/outcome drivenapproach in implementing the nursing plan of care. Maintain contact with patients, physicians, clinicalmanager(s), other members of the healthcare team in a timely mannerregarding patient/family needs and status changes. Participate incare coordination activities and discharge planning asappropriate. Maintain the highest standards of professional conduct inrelation to information that is confidential in nature. Shareinformation only when the recipient’s right to access is clearlyestablished and the sharing of such information is dearly in thebest interests of the patient. Appropriately communicate to ensure adherence to professionalstandards in the provision of and availability of supplies,materials and equipment needed to safely and effectively implementthe plan of care. Prepare, submit and maintain documentation as required by theCompany and/or facility. Visit/shift notes documented on dayservices are rendered.

Required Experience/SkillsUse your skills tomake an impact Graduate of an accredited Licensed Practical Nursing Program oraccredited School of Vocational Nursing. Current nursing license in the practicing state. Valid drivers license, auto insurance and reliabletransportation. Current CPR certification. Two years experience as an LPN/LVN in a clinical setting,preferably in a home health or hospice setting.

  • $61,400 per year base payScheduled Weekly Hours40PayRangeThe compensation range below reflects a good faith estimate ofstarting base pay for full time (40 hours per week) employment atthe time of posting.
