SNF & Rehab Seeking a dedicated LPN to provide compassionate care in a secure Alzheimer’s and Dementia care setting. This role requires working on Every Other Weekend, with varied shifts Days/Nights/Evenings.
Responsibilities:
Deliver hands-on care, including medication administration and treatments Monitor and report changes in resident health Support residents with activities of daily living (ADLs) Maintain accurate documentation Collaborate with families and interdisciplinary teams Follow infection control and safety procedures
Requirements:
Active LPN license in good standing Experience in skilled nursing, rehab, or long-term care preferred Strong clinical and communication skills #LPN1
