42635032 May be Eligible for up to a $2,000 Sign-on Bonus Provide nursing services to patients and families within the scope of practice of the LPN license as defined by the State of New Hampshire Board of Nursing. Meet or exceed standards of performance as established for employment at DHK with strong focus on exceptional customer service. Efficiently gathers information from patients and/or caregivers in person or on the phone and provides telephone nursing care. Resources available for telephone care include departmental guidelines, DHK standard telephone triage books, and provider specific information. The LPN recognizes their limits of knowledge and experience and plans for management of situations beyond their expertise, using provider and RN available resources in the department. Review medical record of scheduled patients to verify that current lab, x-ray or procedure reports are available for provider and fill out forms required for appointment. Accurately performs required computer functions including but not limited to medication reconciliation, updating past medical and surgical history problem lists in the EMR, chronic health maintenance, scheduling of appointments including order entry, and management of task lists. Escorts patient to exam room and prepare for examination/procedure being aware of age, language barriers, and physical limitations. Assist provider with exams and procedures as necessary. Prepare, administer, document and submit charges for immunizations or medications and treatments under provider direction. Any lab specimens obtained sent to the lab with all necessary paperwork or computerized order entry completed and the specimens properly labeled. Point of Care testing obtained, documented, and communicated to appropriate provider. Accurately document patient interactions, including but not limited to calling or transmitting of prescriptions, notification of test results, nursing advice. Obtain prior authorization or pre-certification as required. Graduate of accredited practical nursing program. 0-2 years experience as a LPN required. Must maintain NH LPN License. BLS within 3 months of hire and maintained on a bi-annual basis.
