LPN Job Responsibilities & Duties:

Direct thedaily activities of the LNAs in accordance with current federal,state and local regulations & guidelines and established facilitypolicies and procedures. Ensure that policies and procedures arecomplied with by nursing personnel assigned. Participate in theadmission, discharge and transfers of residents as required.

Observe for changes in residents status, notifying the physicianand residents family or representative and documenting accordingly.

Transcribe physician orders to medical record and carries outorders as written. Prepare and administers medications as perphysicians orders & observes for adverse effects. Perform woundtreatments per physicians orders, observe and document accordingly.

Schedule follow up appointments for residents and transportationneeds as indicated. Provides nursing leadership to nursingpersonnel assigned to the unit. Provides direct care skills such ascolostomy changes, tube feedings, wound care, suctioning, IVadministration, etc. in accordance with policies and procedures.

Collaborates with other members of the interdisciplinary team.

Informs appropriate departments of concerns or complaints and filesgrievances as necessary. Initiates, reviews and updates care plans.

Participates in survey process and in end-of-life care as required.

Ensures medications, supplies, and equipment are stocked. Maintainconfidentiality of protected health information, including verbal,written and electronic communications.
