  • Verifies the Plan of Care prior toeach visit and provides care according to delegation by the RN,physician’s orders, assessment data, and established standards andguidelines.
  • Incorporates patient care goals established in thePlan of Care, into nursing visits, as evidenced by documentation onskilled nursing notes.
  • Communicates significant findings,problems and changes in condition or environment to the RN CaseManager, the physician, clinical supervisor, and/or other personnelinvolved with patient care.
  • Reports unsafe conditions to theappropriate RN Case Manager, clinical supervisor, and physician, asappropriate.
  • Implements the plan for patient safety, using thepatient, family, and community resources.
  • Assists the patient inlearning appropriate self-care techniques while delivering qualitypatient care.
  • Suggests revisions to the plan of care in responseto identified patient care issues and notifies the RN Case Manager
  • Provides those services required substantial and specializednursing skills under the direction of a Registered Nurse orphysician.
  • Collaborate with the RN Case Manager during normalbusiness hours for advice, assistance, and instructions.
  • Documents medication regimen and updates medication profileaccording to established procedures.
  • Treats patients andcaregivers in the highest and most effective manner.
  • Implementall available actions to prevent avoidable hospitalizations and ERvisits.
  • Provides positive, supportive communication tophysicians, patients, families, visitors, and agency personnel.

What is Required?

  • Graduate of an accredited Certificate, Diploma,or Associate School of Nursing
  • Current State License as aLicensed Vocational / Practical Nurse
  • 1 year of experience in aclinical care setting
  • Experience in a hospice or home healthenvironment is preferred
  • 50% travel required
  • Sit, stand, bend,lift and move intermittently and be able to lift 50-100 lbs.
  • Valid driver’s license and insurance and reliable transportation toperform job tasks You will report to the Clinical Team Manager.
