RN and LPN
- Per Diem RiverWoods Manchester is seekingPer Diem RN and LPN staff for overnight (11p-7a) shifts to providedirect resident care in a team

Responsibilities:
- Performs professional duties in accordance withfacility policies/procedures and standards of professionalpractice, State of New Hampshire Nurse Practice Act, OBRA andHIPAA.
- Collaborates with interdisciplinary team members toformulate individualized resident care plans. Updates care plans asnecessary. Participates in Resident Care Plan Meetings.
- Institutes care protocols as indicated including: falls prevention,safety and elopement, behavior management, weight management, skinintegrity, bowel/bladder management, infection control guidelines(Standard Precautions), and pain assessment and management.
- Assesses and documents changes in residents’ physical or mentalstatus and reports findings to the primary care physician andtreatment team. Provides complete clinical data to facilitatedecision-making.
- Provides leadership and direction for directcaregivers. Prepares daily assignments and delegates workloadappropriately. Supervises direct care through observation andevaluation of direct care results.
- Establishes positive workingrelationships with team members, residents and family members inorder to facilitate open, direct communication.
- Obtains,administers and documents medications and treatments given toresidents following professional guidelines and Nursing departmentpolicies. Inventories and maintains locked system for narcotics.
- Supports, creates, and maintains a positive culture for teammembers and residents
Qualifications:
- Must possess, as a minimum,a Nursing Degree or Diploma from an accredited college, Universityor nursing school.
- Long term care facility and specializedknowledge of nursing care of the elderly and rehabilitative nursingis preferred.
- Must hold a registered nursing (RN) license withinthe State of New Hampshire.
- Certified in intravenousadministration preferred.
- Must maintain CEU’s annually.
