Experienced LPN for Skilled Nursing Facility 3.4 3.4 out of 5 stars Peterborough, NH 03458 We are seeking a compassionate and skilled Licensed Practical Nurse for our facility partners in Peterborough, NH.
RESPONSIBILITIES
Collect, report, and document patient data; monitor condition changes. Assist in developing and implementing care plans to meet patient goals. Administer medications and treatments per physician orders. Accurately document care and communicate updates to RN/physician. Support discharge planning and provide patient/family education. Assign, monitor, and support tasks performed by unlicensed staff. Use PointClickCare (PCC) system for documentation. Perform drug counts and assist physicians during rounds. Collaborate with other departments for effective patient care. Attend required trainings and help onboard new staff. Promote a safe, respectful, and confidential care environment. Stay beyond scheduled shifts as needed. Perform other duties as assigned.
QUALIFICATIONS
Valid state LPN license
- NH Min 1 yr LPN experience
- SNF or similar setting. Strong patient care and assessment skills BLS (must be AHA
