out of 5 stars Franklin, NH 03235 A renowned healthcare system in New Hampshire is searching for a rehabilitation and transition nurse to join their growing team! In this role, you will provide short-term subacute skilled care and rehabilitation services to patients transitioning from acute hospital stays back to their homes. You’ll collaborate closely with an interdisciplinary team of nursing, physical therapy, occupational therapy, and other support services to deliver holistic, patient-centered care.

Responsibilities:

Assess, plan, implement, and evaluate nursing care based on patient needs Use clinical judgment and critical thinking to guide interventions and decision-making Conduct patient assessments and interpret data to tailor individual care plans Document patient care and monitor progress toward treatment goals Educate patients and families on care plans and post-discharge instructions

Qualifications:

Active NH RN or LPN license. 1+ years of experience.
