Incorporates patient care goals established in the Plan ofCare, into nursing visits, as evidenced by documentation on skillednursing notes. Communicates significant findings, problems and changes incondition or environment to the RN Case Manager, the physician,clinical supervisor, and/or other personnel involved with patientcare. Reports unsafe conditions to the appropriate RN Case Manager,clinical supervisor, and physician, as appropriate. Implements the plan for patient safety, using the patient,family, and community resources. Assists the patient in learning appropriate self-caretechniques while delivering quality patient care. Suggests revisions to the plan of care in response toidentified patient care issues and notifies the RN CaseManager. Provides those services required substantial and specializednursing skills under the direction of a Registered Nurse orphysician. Collaborate with the RN Case Manager during normal businesshours for advice, assistance, and instructions. Documents medication regimen and updates medication profileaccording to established procedures. Treats patients and caregivers in the highest and mosteffective manner. Implement all available actions to prevent avoidablehospitalizations and ER visits. Provides positive, supportive communication to physicians,patients, families, visitors, and agency personnel.

What isRequired? Graduate of an accredited Certificate, Diploma, or AssociateSchool of Nursing Current State License as a Licensed Vocational / PracticalNurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment ispreferred 50% travel required Sit, stand, bend, lift and move intermittently and be able tolift 50-100 lbs. Valid driver’s license and insurance and reliabletransportation to perform job tasksYou will report to the ClinicalTeam Manager.
