RN/LPN Full Time 40 hours a week 3-11 $3000 Sign onBonus RiverWoods Exeter is currently seeking a Full Time 40hour aweek RN or LPN to provide direct resident care in a team-focusedenvironment where our residents are at the heart of what we do.

Responsibilities:
Performsprofessional duties in accordance with facility policies/proceduresand standards of professional practice, State of New HampshireNurse Practice Act, OBRA and HIPAA. Collaborates withinterdisciplinary team members to formulate individualized residentcare plans. Updates care plans as necessary. Participates inResident Care Plan Meetings.
Institutes care protocols as indicatedincluding:
falls prevention, safety and elopement, behaviormanagement, weight management, skin integrity, bowel/bladdermanagement, infection control guidelines (Standard Precautions),and pain assessment and management. Assesses and documents changesin residents’ physical or mental status and reports findings to theprimary care physician and treatment team. Provides completeclinical data to facilitate decision-making. Provides leadershipand direction for direct caregivers. Prepares daily assignments anddelegates workload appropriately. Supervises direct care throughobservation and evaluation of direct care results. Establishespositive working relationships with team members, residents andfamily members in order to facilitate open, direct communication.
Obtains, administers and documents medications and treatments givento residents following professional guidelines and Nursingdepartment policies. Inventories and maintains locked system fornarcotics. Supports, creates, and maintains a positive culture forteam members and residents
Qualifications:
Must possess, as aminimum, a Nursing Degree or Diploma from an accredited college,University or nursing school. Long term care facility andspecialized knowledge of nursing care of the elderly andrehabilitative nursing is preferred. Must hold a registered nursing(RN, LPN) license within the State of New Hampshire. Certified inintravenous administration preferred. Must maintain CEU’s annually.
