SCOPE OF POSITION

Ensures that the day-to-day wellness needs of Ernest P. Barka Assisted Living Community residents are met by carrying out personal care treatments and medication administration. Acting as part of an interdisciplinary team, Wellness Coordinators ensure residents receive consistent, high-quality services and help residents achieve their long and short-term wellness goals. The Wellness Coordinator acts as a resource for residents, families, and providers and is encouraged to actively build authentic relationships as part of the overall mission of delivering person-centered care.

A DAY IN THE LIFE

Promotes the mission, vision and values of the community. Complies with established nursing standards, policies, and procedures of the Ernest P. Barka Assisted Living Community. Maintains residents’ privacy, dignity, and confidentiality. Ensures residents are treated with respect and dignity and ensures quality care as residents’ healthcare needs change. Follows up on issues identified on each shift. Attends all mandatory in-service training as required and other in-services within and outside the facility to upgrade nursing skills and improve general knowledge. Communicates with Wellness Coordinator in a timely manner for any significant issues i.e. change in residents’ condition, environment, or personnel concerns. Performs and signs off on narcotic count with oncoming and outgoing shifts. Receives report from outgoing shift and gives report to oncoming shift. Writes an assignment for Wellness personnel on duty. Carries out physicians’ orders and treatments. Follows care plans developed by the Wellness Manager. Administers medication and treatments and completes documentation in accordance with Ernest P Barka Assisted Living Community policies. Responsible for checking accuracy on monthly MAR. Initiates and assists with admission and documentation of new residents. Identifies and utilizes infection control techniques at all times in accordance with Ernest P Barka Assisted Living Community’s universal and blood borne pathogen policy. Monitors and participates in Quality Improvement and Risk Management programs at Collaborative Care Meetings and Family Care Conferences. Interacts with residents and various family members, physicians, health service agencies to maintain positive communication and relationships. Follow-up on family issues as directed by Wellness Manager. Participates in community events, including family and social functions. Be available to meet with residents to discuss health and wellness and to encourage participation in exercise programs when deemed appropriate. Oversees the availability and replenishment of unit supplies as needed. Attends mandatory in-services trainings annually. Takes vital signs, temperature, blood pressure, pulse and respiration. Initiates individualized personal service plans and proactively manages care and services for each resident. Assists with resident comfort. Encourages teamwork through cooperative interactions with all departments to assist in eliminating / correcting problem areas and overall improvement of services. Helps coach staff as necessary. Contributes to team effort by volunteering to helping others to accomplish department and community goals in a caring positive manner to maintain the residents.

REQUIRED EDUCATION AND EXPERIENCE

Minimum of three (3) years full-time or equivalent experience in a Health Care setting with a minimum of one (1) year in a supervisory role; or An equivalent combination of work and experience will be considered. Licensed nurse in the state of New Hampshire (LPN/RN) required.
