FULL-TIME PART-TIME

Second shift – Flexible shift hours The position includes caring for the clinical needs of our residents in memory care and assisted living, maintaining resident records, and coordinating with the care team members including residents, families, providers, and pharmacy. We utilize PointClickCare for all of our electronic medical records. _____________________________________________________________________________ Responsibilities include but are not limited to:: Record medical information. Provide basic care to patients. Administer prescribed medications. Assist with care coordination. Prepare patients for examination or treatment. Assist with personal care and household chores.

Qualifications:

MUST have a valid LPN license in the state of New Hampshire Strong knowledge of nursing principles and practices Must be a team player Experience managing staff
