Job Description

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Serenity Hospice Care is looking for an organized and dedicated Clinical Administrative LPN to join our team. The Clinical Administrative LPN plays a vital role in supporting the clinical and administrative operations of the hospice team. This position ensures the efficient coordination of patient care activities, documentation, scheduling, and communication between interdisciplinary team members, patients, and families.

Responsibilities:

+ Review clinical documentation for accuracy, completeness, and compliance with hospice policies and Medicare guidelines. + Support triage functions by responding to patient/family calls and communicating with field staff and providers. + Schedule nursing visits, on-call coverage, and follow-up care in coordination with field staff and leadership. + Prepare reports for quality assurance and performance improvement (QAPI) initiatives. + Assist with audits, surveys, and internal reviews to ensure adherence to regulatory and organizational standards. + Participate in the preparation of the interdisciplinary team (IDT) meetings, ensuring updated patient records and reports are available. + Monitor and communicate any concerns related to clinical documentation, patient status, or care coordination. + Act as a liaison between physicians, hospice staff, patients, and family members to facilitate effective communication and continuity of care. + Coordinate communication related to medication orders, DME, supplies, and hospice services with appropriate vendors and partners.

Requirements:

+ Current LPN license in the state of New Jersey (required). + Minimum of 2 years of clinical nursing experience; hospice or home health experience preferred. + Experience in a healthcare administrative or coordination role is highly desirable. + Knowledge of hospice regulations and Medicare/Medicaid guidelines. + Proficiency in EMR systems and Microsoft Office Suite. + Strong organizational skills and attention to detail. + Excellent verbal and written communication skills. + Ability to work independently and collaboratively in a team environment. + Compassionate, empathetic, and respectful in all patient and family interactions.
