Schedule:

3 12-hourshifts each week including alternating weekends What We offer:

Provide direct patientcare as defined in State Nurse Practice Act Implement currentnursing practice following a comprehensive assessment and the planof care Provide accurate and timely documentation of patientservices to reflect the plan of care Assess and provide patient andfamily/caregiver education and information pertinent to diagnosisand self-plan of care Participate in the coordination of hospiceservices, appropriately reporting the identified needs to theinterdisciplinary group. Provide appropriate pain/symptommanagement. Evaluates the patient’s response totreatments/medications

Qualifications:

Graduate of an accreditedpractical nurse or a vocational nursing program Three years ofnursing experience.
