Nursing Full Job Description Job Summary:

We are seeking a compassionate and experienced LPN to join our Home Health team. The successful candidate will provide high-quality care to patients in their homes, focusing on medical-surgical care, trauma medicine, and infection control. The ideal candidate will have a strong background in pediatric home health, be skilled in medical documentation, and possess a passion for delivering care that exceeds expectations.

Scheduling needs:

Shifts available:

WEDNESDAY, FRIDAY AND SATURDAY

7am-7pm

Responsibilities:

• Provide skilled nursing care to patients in their homes.
• Conduct thorough assessments of patients to identify and address their medical, emotional, and social needs.
• Administer medications and treatments as prescribed by physicians, and monitor patient response.
• Document patient care in accordance with agency policies and standards.
• Provide care, including administering medications, feeding, and bathing, as needed.
• Maintain accurate and up-to-date medical records, including medical documentation and reporting of patient outcomes.
• This is a school case so must accompany patient to school.

Qualifications:

• Current LPN license/RN license in the state of New Jersey.
• Minimum of 2 year of experience as an LPN, preferably in home health or a related field.
• Excellent communication and interpersonal skills.
• Experience with Trach, G Tube and Vent preferred.
