Job Summary:

We are seeking a compassionate and experienced LPN/RN to join our Home Health team.

Scheduling needs:

Tuesday and Thursday 6:30am to 3:00pm & Saturdays 9am to 9pm The patient goes to school on Tuesdays and Thursdays so the nurse on this case will have to accompany the patient to school.

Responsibilities:

• Provide nursing care to patients in their homes.
• Conduct thorough assessments of patients to identify and address their medical, emotional, and social needs.
• Educate patients and their families on medical conditions, treatments, and self-care techniques.
• Provide toddler care, including administering medications, feeding, and bathing, as needed.
• Maintain accurate and up-to-date medical records, including medical documentation and reporting of patient outcomes.
• Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
• Maintain a safe and healthy environment, adhering to infection control practices and promoting patient safety.

Qualifications:

• Current LPN/RN license in the state of New Jersey.
• Minimum of 1 year of experience as an LPN, preferably in home health or a related field.
• Experience with Trach and Vent preferred.
