Summary As an LPN – Home Health with 1st Americare, you will provide high-quality nursing care to patients in their homes. This role is vital in ensuring patient comfort and health while contributing to our mission of delivering exceptional healthcare services. Shift are Friday 11pm-7am Responsibilities Conduct comprehensive assessments of patients’ health status and needs. Administer medications and treatments as prescribed by physicians. Manage medical records and documentation accurately and timely. Provide education and support to patients and their families regarding health management. Collaborate with interdisciplinary teams to develop and implement care plans. Monitor patient progress and adjust care plans as necessary. Ensure compliance with healthcare standards and regulations. Utilize critical thinking skills to respond to emergencies effectively. Qualifications Valid LPN license in the state of practice. Strong knowledge of anatomy, physiology, and airway management techniques. Experience with medical records management is essential. Excellent communication skills for effective patient interaction. Demonstrated ability to work independently and manage time effectively. Commitment to maintaining high standards of patient care. Experience with g-tube care .
