Key Responsibilities:

Provide outstandingcustomer service to all residents, family members, responsiblepersons, and visitors to the residence Coordinate and delivernursing care to residents within the scope of an LPN, including butnot limited to catheter care, wound care, dressing changes,nebulizer treatments, ostomy care, and management of medicalequipment; medication management including administration of oralmedications, injections or PRN medications. Supervise the careprovided by HHAs and RCAs to residents. Manage and administermedications and assist the residents with self-administration ofmedication. Maintain positive collaborative relations withresidents, families and provider partners Report any observedsignificant incidents and/or changes in the resident’s condition toNursing Leadership (Charge RN in the Residence), Primary CareProvider and family as indicated. Review and become familiar witheach assigned resident’s Individualized Service Plan. Assist intraining and orientation of Resident Care staff. Document care andservices provided to residents in the electronic health recordusing nursing best practices and documentation standards. Followall Insp?r policies and procedures as outlined in the policy andprocedure manual Attend and participate in scheduled in-serviceprograms, training programs and staff meetings. Assist withscheduling and staffing of Resident Care staff as needed. Providecoaching and mentoring to Resident Care Team members as needed.

Follow emergency procedures. Understand and comply with all Insp?rsafety practices and procedures. Participate in health, wellnessand lifestyle programs and assist with all aspects of resident careand resident experience as needed May perform other duties asassigned.

Education/Experience/Licensure/Certification:

Must be aLicensed Practical Nurse, licensed to practice in the state of NewYork with one of the following: Associates degree and a minimum oftwo (2) years of full time, or the equivalent of full time,clinical experience in nursing, at least one (1) year of whichshall be in a home health care agency, hospital, skilled nursingfacility, or community health program that included care of thesick at home A diploma or associates degree in nursing and at leastfour (4) years of full-time, or the equivalent of full-time,clinical experience in nursing within the past ten (10) years, atleast one (1) year of which shall be in a home health care agency,hospital, skilled nursing facility, or community health programthat included care of the sick at home Basic first aid and CPRcertification. Experience in assisted living, post-acute orlong-term care setting preferred. Experience with and/or specialinterest in working with seniors. Why You’ll Love working for
