Job Description Within the context of the Nursing ProfessionalPractice Framework, the LPN provides quality and cost-effectivedirect care for patients/families within a unit, service or otherprimary area of practice.

• Collaborates with members of healthcare team to assist patients in achieving established outcomes ofcare
• Participates in direct patient care in order to meetindividual and family health goals by: obtaining, documenting andreporting patient data related to established plan of care
• Performs therapeutic measures, under the direction of an RN or MD,to assist in achieving established patient care outcomes
• Travelwithin Hartford Region offices including Glastonbury andFarmington. Qualifications
• Current Connecticut Licensed PracticalNurse license required.
• Medical/Surgical office experiencepreferred.
