Job Responsibilities:

Provide one-on-one home care nursing as a skilled Licensed Practical Nurse (LPN) Administer medications, infusions, and injections in a home care setting Monitor and assess patient conditions, documenting progress Collaborate with families and healthcare teams to develop care plans Educate patients and caregivers on proper care techniques

Qualifications:

Active Licensed Practical Nurse (LPN) license in New York At least 2 years of nursing experience (home care LPN experience preferred) Experience with Trach, Vent, Tube feeding, or Infusions preferred Strong clinical skills and a passion for patient-centered care Ability to work independently and adapt to different home care environments Pediatric experience preferred but not required Apply Today!
