SUMMARY:

As a Licensed Practical Nurse, you willprovide professional patient care to long-term and post-acute careresidents. You will direct and monitor the care provided by CNAsand consistently ensure proper documentation and recording istaking place. Your ability to lead is crucial in both the successof the CNAs and the care of the patients. Attached to CliftonSprings Hospital, Clifton Springs Nursing Home offers a uniquelyholistic approach to long-term care. Skilled nursing care islocated adjacent to Spa Springs, where patients can access massagetherapy, acupuncture and similar treatments. Clifton SpringsNursing Home is a 108-bed facility.

RESPONSIBILITIES

• Responsiblefor timely, accurate medication administration of assigned team andconcurrent required documentation.
• Performs treatments anddocuments progress of same as per physician orders and facilityprotocol.
• Assures concise, pertinent and complete resident caredocumentation using computerized medical record process. Assuresthe initiation of appropriate care pathway tools.
• Responsible forreceiving/giving relevant reports, both written and verbal from/toappropriate staff as per departmental protocols.
• Assures propermonitoring of residents and relevant condition status are reportedto charge nurse/supervisor and/or physicians in a timely manner.
• Completes pre-reviews of medication administration records as perfacility protocols.
• Completes required MDS documentation and allrelated observations in a timely manner.
• Supports CNA teamthrough assisting in direct ADL care of residents.

REQUIREDQUALIFICATIONS

• Graduate from an accredited School of PracticalNursing.
• Current licensure in State of New York.

PREFERREDQUALIFICATIONS

• One year experience preferred.
