These positions are part-time, within the Nursing Department, working across all JRCHC medical sites. LPN – Part-time/Flexible Jericho Road Community Health Center is seeking LPNs who are interested in part-time hours with very flexible scheduling. You will have the ability to design your schedule, in conjunction with nursing leadership, within our hours of operation.

Responsibilities:

Provide direct patient care from birth on in a busy FQHC Demonstrate knowledge of current standards of practice and perform current nursing practice expectations within established guidelines. Work collaboratively with clinician (MD, NP, PA, CNM, DO) to provide excellent clinical care. Collect samples (i.e. blood, urine, stool or sputum) from patients and perform routine laboratory tests (i.e. lead, hgb, glucose, strep) Prepare patients for examinations, testing, or treatments for all visit types Perform EKG and spirometry Perform daily pre visit planning with assigned medical provider per protocol Administer vaccinations according to NYS and CDC guidelines for adult and pediatric patients Administer IM medications as ordered (Toradol, B12, Phenergan, Cetriaxone, etc) Conduct telephonic communication with patients regarding test results and follow up care needs Maintain a clean work space and stocked and cleaned exam rooms. Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state/local regulations, and patient satisfaction. Complete all patient charting and ensure all patient records are current/complete with strong attention to detail and accuracy.

Qualifications:

Graduation from an accredited nursing program with a current NYS state licensure as a LPN. Direct experience in primary care (preferred but not required). Reliable transportation between sites required. Current CPR and Basic Life Support Certification (BLS)

Hourly Rate:

$26.00-28.00 an hour. Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components. Registered Nurse – Part-time/Flexible Hours (remote option upon successful completion of 90 day introductory period) Jericho Road Community Health Center is seeking Community Health RNs who are interested in part-time hours with flexible scheduling. This opportunity has the potential to be a remote position upon the successful completion of a 90 day introductory period.

Responsibilities:

Perform physical exams and patient assessments to identify and address health problems and unmet patient care needs. Triage patient concerns and immediate needs by phone or in person. Provide nursing intervention and health education for individuals, families, and/or other groups regarding disease prevention, childbirth, support programs, and other health-related concerns. Perform medical treatments (medication and vaccine administration) as prescribed by a physician, nurse practitioner, physician assistant or midwife. Collect samples (blood, urine, stool or sputum) from patients and perform routine laboratory tests. Perform EKG and Spirometry Complete all patient charting and ensure all patient records are current/complete with strong attention to detail and accuracy Provide Hospital and ER follow up for patients recently discharged from hospitalization and assist in transition of care.

Qualifications:

Graduation from an accredited nursing program with a current NY state licensure as an RN Hybrid Option available after probationary period Direct experience in a primary care (preferred but not required) Reliable transportation between sites required. Current CPR and Basic Life Support certification (BLS) Maintain credentials, attend seminars, and fulfill all licensure requirements including continuing education.

Hourly Rate:

$32-$35 an hour. Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
