Job Summary The Licensed Practical Nurse (LPN) provides generalnursing care to patients, families and caregivers in the MedicalGroup practices. The Licensed Practical Nurse (LPN) assists andadvocates for the patient and families health in support of thepatient centered approach and delivery model of care, and is anactive care team member. Responsible for collaborating withphysicians/ providers , registered professional nurses andmultidisciplinary care team members. A Licensed Practical Nurse(LPN) can perform tasks and responsibilities within the frameworkof case finding, health coaching, and supportive care under thedirection of the registered professional nurse or licensedphysician or licensed health care provider. Nursing shall performin accordance with the standards outlined for Licensed PracticalNurse (LPN) in the NYS Education Law, NYS Office of theProfessions, and by the State Nurse Practice Act. A LicensedPractical Nurse will follow the mission, vision, I-CARE values,policies and procedures of Mohawk Valley Health System. Core JobResponsibilities
- Gather patient measurements and collect patientdata
- Record and report patient findings
- Prepare patient forpatient examination and treatment
- Assistswith examination andtreatments
- Perform tasks within the framework of supportive care,under the clinical direction of licensed health care provider orregistered professional nurse
- Support self-care , self-management and the established plan of care.
- Utilize standardprecautions and practice at all times
- Maintain equipment,supplies, inventory, and a safe and clean work environment
- Maintain technical knowledge by attending educational workshops,mandatory in-services; reviewing publications and participating indepartment committee meetings or work groups
- Engage in qualityimprovement initiatives including continuing education,participation in system-wide and department specific teams
- Maintain a cooperative relationship among the healthcare team byeffectively communicating information
- Respond to patientrequests, communicates effectively to the licensed health careprovider who can fulfill the patient request
Education/ExperienceRequirementsRequired:
- Graduate of an accredited school oflicensed practical nursing program
Preferred:
- Bachelors orMasters of Science in Nursing from accredited nursing program
Licensure/Certification Requirements Required:
- Current New YorkState licensure as a Licensed Practical Nurse;
- Basic Life SupportCertification
