The Residential LPN will: Assist the Registered Nurse (RN) in coordinating following through on provisions of healthcare services Keep progress notes and summarize for quarterly, semi-annual and annual reviews Provide healthcare to individuals receiving services; provide direct hygiene and health instruction to individuals receiving services and families, as required Handle control of communicable diseases and infections through identification and assessment Provide first aid to persons in need of immediate care Assist the Physicians in medical examinations, as needed Schedule appointments and follow up on Physicians’ recommendations Keep up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data via progress notes Administer medications when appropriate. Perform other related duties, as requested

Qualifications:

1+ year of experience as Licensed Practical Nurse (LPN) Bachelor’s Degree NYS Licensed Practical Nurse (LPN) CPR certification Experience in the delivery of community-based health services to individuals with Developmental Disabilities Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices Solid assessment and documentation skills Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized
