Essential Functions Cultivate relationships with our physicians and hospitals to facilitate the best Home Care solutions for their patients. Monitor HCR RightFax folders, label faxes and attach electronic documents to patient record in Home Care Home Base (HCHB). Monitor Intake Team email. Produce timely, accurate data entry. Responsible for entering orthopedic prescreens into HCHB according to agency protocol. Provide phone support to the department and triage of calls to Intake staff. Provide excellent customer service to internal and external customers. Participate in department and agency meetings. Other duties as assigned. Education Requirements Associate’s Degree required. Qualifications and Requirements Minimum of 4 years experience in a clinical setting. Exceptional customer services and communication skills required. Must be highly proficient and efficient working with a computer, and be detail oriented and able to work in a fast-paced environment. Work Environment The Client Relationship Specialist is primarily in an office setting and may be exposed to outdoor conditions.
