SUMMARY:

As a Licensed Practical Nurse, you willprovide professional patient care to long-term and post-acute careresidents. You will direct and monitor the care provided by CNAsand consistently ensure proper documentation and recording istaking place. Your ability to lead is crucial in both the successof the CNAs and the care of the patients. DeMay Living Center,connected to Newark-Wayne Community Hospital is a 180 bed skillednursing facility providing 24-hour care to people and theirfamilies in need of rehabilitative, complex post-acute,transitional, neurobehavioral and long term care. We also provide asafe, comforting setting for people experiencing symptoms ofdementia.

RESPONSIBILITIES
  • Responsible for timely, accuratemedication administration of assigned team and concurrent requireddocumentation.
  • Performs treatments and documents progress of sameas per physician orders and facility protocol.
  • Assures concise,pertinent and complete resident care documentation usingcomputerized medical record process. Assures the initiation ofappropriate care pathway tools.
  • Responsible for receiving/givingrelevant reports, both written and verbal from/to appropriate staffas per departmental protocols.
  • Assures proper monitoring ofresidents and relevant condition status are reported to chargenurse/supervisor and/or physicians in a timely manner.
  • Completespre-reviews of medication administration records as per facilityprotocols.
  • Completes required MDS documentation and all relatedobservations in a timely manner.
  • Supports CNA team throughassisting in direct ADL care of residents.
REQUIRED QUALIFICATIONS
  • Graduate from an accredited School of Practical Nursing.
  • Current licensure in State of New York.
PREFERRED QUALIFICATIONS
  • One year experience preferred.

  • Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25pounds of force frequently, and/or greater than negligible up to 10pounds of force constantly to move objects; Requires frequentwalking, standing or squatting.
