D/E/N with rotating every otherweekend SUMMARY The Licensed Practical Nurse is responsible forproviding direct patient care, usually under the direction of aRegistered Nurse, in accordance with established plans.

As the area’s only PACE program, ElderONErepresents the future of senior health care. This long-term careprogram provides the entire continuum of medical care and supportservices to seniors 55 and older with chronic care needs, allowingthem to maintain their independence in the comfort of their homewithout compromising their health or safety. RESPONSIBILITIESPatient Care & Service. Promote and restore patients’ health bycompleting the nursing process; collaborate with physicians andmultidisciplinary team members; perform various treatmentprocedures; provide physical, educational and emotional support topatients, friends and families; supervise assigned team membersDocumentation. Ensure concise, pertinent and complete resident caredocumentation using computerized medical record process MedicationAdministration & Reporting. Administer medication and give/receiverelevant reports, both written and verbal from/to appropriate staffas per departmental protocols

Required Licensure/CertificationSkills:

• Current licensure in the State of New York
