Job Summary The Speciaty Licensed Practical Nurse providesgeneral nursing care to patients, families and caregivers in theMedical Group practices. The Licensed Practical Nurse assists andadvocates for the patient and families health in support of thepatient centered approach and delivery model of care, and is anactive care team member. Responsible for collaborating withphysicians/ providers , registered professional nurses andmultidisciplinary care team members. A Licensed Practical Nurse canperform tasks and responsibilities within the framework of casefinding, health coaching, and supportive care under the directionof the registered professional nurse or licensed physician orlicensed health care provider. Nursing shall perform in accordancewith the standards outlined for Licensed Practical Nurse in the NYSEducation Law, NYS Office of the Professions, and by the StateNurse Practice Act. A Licensed Practical Nurse will follow themission, vision, I-CARE values, policies and procedures of MohawkValley Health System. Core Job Responsibilities

• Gather patientmeasurements and collect patient data
• Record and report patientfindings
• Prepare patient for patient examination and treatment
• Assistswith examination and treatments
• Perform tasks within theframework of supportive care, under the clinical direction oflicensed health care provider or registered professional nurse
• Support self-care , self
• management and the established plan ofcare.
• Utilize standard precautions and practice at all times
• Maintain equipment, supplies, inventory, and a safe and clean workenvironment
• Maintain technical knowledge by attending educationalworkshops, mandatory in-services; reviewing publications andparticipating in department committee meetings or work groups
• Engage in quality improvement initiatives including continuingeducation, participation in system-wide and department specificteams
• Maintain a cooperative relationship among the healthcareteam by effectively communicating information
• Respond to patientrequests, communicates effectively to the licensed health careprovider who can fulfill the patient request

Education/ExperienceRequirements Required:

• Graduate of an accredited school oflicensed practical nursing program

Preferred:

• Bachelors orMasters of Science in Nursing from accredited nursing program

Licensure/Certification Requirements Required:

• Current New YorkState licensure as a Licensed Practical Nurse
• Basic Life SupportCertification
