Job Summary The Licensed Practical Nurse

• NYSUT (VNA) isresponsible for performing tasks and responsibilities within theframework of health teaching, health counseling, and provision ofsupportive and restorative care as directed by the RegisteredProfessional Nurse. Core Job Responsibilities
• Assists inprovision of nursing care to the home health patient. Collaborateswith the patient’s primary case manager and other members of thehealth care team. Makes and documents home care nursing visits.
• Gathers data and reports significant change in patient’s conditionto Registered Nurse or Supervisor as appropriate promptly.
• Instructs patients and caregivers according to the plan of care,implements orders for medications and treatments as prescribed bythe physician.
• Promotes good health practices, helping thepatient in accepting and assuming responsibility for attaining thehighest level of independent functioning possible.
• Completes,maintains and submits accurate, legible, and relevant nursingdocumentation using established agency abbreviations regarding thepatient’s condition and all care and treatment given.
• Monitorspatient expectations and satisfaction; participates in patient careconferences as appropriate.
• Accepts assignments and performsduties within the limits of preparation and experience, seeking andobtaining sufficient information necessary to function effectivelyin the home health care setting.
• Assumes responsibility tomaintain and upgrade professional knowledge and skills throughattendance and participation in continuing education, in-serviceprograms, professional conferences and community and agencycommittees.
• Participates in appropriate agency meetings and thequality improvement and utilization review activities of theorganization.
• Supports the Agency’s Compliance Program, ensuringthat all information is kept confidential and is respectful of thePatient’s Bill of Rights.
• Complies with all MVHS and VNA policiesand procedures, as well as applicable regulations.
• Is flexible towork a varied schedule based on patient needs.
• Performs otherduties as required.

Education/Experience Requirements

REQUIRED

• At least two years of experience working as a Licensed PracticalNurse, preferably in acute care setting.
• Excellent written,verbal and interpersonal communication skills.
• Proficient withEHR tools.
• Ability to travel to patient homes which may not behandicap accessible with reliable transportation.

Licensure/Certification Requirements

REQUIRED

• License andcurrent registration to practice as a Licensed Practical Nurse inNew York State.
• Current BLS certification.
