Job Description

• Facilitate communication between the patient andthe interdisciplinary team.
• Develop and maintain a therapeuticrelationship by providing education and promoting self-advocacy andindependence.
• Utilize state-of-the-art technology to documentskilled services provided. Qualifications
• Licensed PracticalNurse with Valid CT LPN license
• One year of LPN experience;Homecare experience preferred.
• Valid driver’s license andreliable transportation required.
• Computer proficiency required.
