The ideal candidate will have experience in various care settings, including clinics, urgent care, and senior care facilities. As an LPN, you will play a vital role in providing high-quality nursing care to patients while working collaboratively with a diverse healthcare team. Responsibilities Provide direct patient care under the supervision of registered nurses and physicians. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Assist in the development and implementation of patient care plans. Monitor patient vital signs and report any changes to the healthcare team. Educate patients and their families on health management and wellness strategies. Document patient information accurately using electronic health record systems such as Athenahealth. Support patients with developmental disabilities by providing tailored care strategies. Collaborate with interdisciplinary teams in teaching hospitals and clinics to enhance patient outcomes. Maintain a clean and safe environment for patients, adhering to infection control standards. Requirements Current Licensed Practical Nurse (LPN) license in good standing. Experience in dementia care, hospice care, or working with individuals with developmental disabilities is preferred. Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare professionals. Ability to work in fast-paced environments such as urgent care or senior care facilities. Knowledge of nursing best practices and familiarity with electronic health records systems like Athenahealth is an advantage.
