Reporting to the Coordinator of Care Nurse, you will be responsible for seeing patients in their home for revisits based on a skilled need
You will implement the plan of care and evaluate the treatment
You will document the visit electronically and completing documentation within 24-48 hours nursing, LPN, homecare, home care licensed practical nurse NY StateLPN(Licensed Practical Nurse) License required
Minimum of one year Med/surge experience in a hospital or Long-term Care facility required
Car and driver license required
