We’re seeking a skilled and dedicatedLicensed Practical Nurse (LPN)to join our collaborative care team in a welcoming outpatient setting. What You’ll Do + Greet and prepare pediatric and adult patients for visits, including taking vitals and updating medical histories + Prepare charts and review records to ensure timely and organized visits + Assist providers during exams, procedures, and treatments + Accurately document patient information in the electronic medical record (EMR) + Respond to routine patient phone messages, escalating concerns to the RN or provider as needed + Process prescription refills and coordinate with pharmacies + Monitor and manage clinic supplies and vendor equipment deliveries + Support infection control protocols and help maintain a safe, welcoming care environment + Participate actively in team meetings and quality improvement efforts Requirements What You’ll Need + CurrentNew York State LPN license + CurrentBLS certification(training can be provided post-hire) + Strong communication, clinical, and organizational skills + Experience with electronic medical records preferred + Commitment to professionalism, teamwork, and confidentiality Why Choose Us? + Daytime Hours Only
