Position Summary:

We are currently seeking a compassionate and reliable Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to join a dedicated healthcare team in Montour Falls, NY . Key Responsibilities:

Provide direct patient care including medication administration, wound care, and vital sign monitoring Assist patients with activities of daily living (ADLs), such as bathing, dressing, and ambulation Accurately document patient observations, interventions, and progress in medical records Report any changes in patient condition promptly to supervising RN or physician Collaborate with physicians, RNs, and other healthcare professionals to implement effective care plans Promote a clean, safe, and supportive environment for patients, families, and staff Offer patient and family education on treatment plans, medications, and health maintenance

Qualifications:

Active LPN or LVN license in the state of New York

• required Graduate of an accredited LPN/LVN program
• required Current CPR/BLS certification
• required Strong communication and interpersonal skills Ability to work both independently and as part of a multidisciplinary team Prior experience in settings such as hospital, long-term care, or home health is preferred, but not required
