These family members believed that appropriate treatment and training in a warm, sheltered, homelike environment would enhance their physical and emotional growth. With the support, funding and licensing of New York State’s Office of People With Developmental Disabilities (OPWDD) and the Office of Mental Health (OMH), ADD came to life. In 2018 ADD was rebranded to RISE Life Services to better reflect the scope and mission of the services the organization now provides. Today RISE operates thirty two homes and multiple different programs and services all around Long Island. We are currently seeking a full time LPN to assist us in our mission of supporting people with developmental disabilities. This is a fantastic opportunity to work in long term care with a select group of individuals. Position would be based in Jamesport or Cutchogue in Eastern Long Island. Under the Supervision of the Director of Nursing, the Licensed Practical Nurse is responsible for the overall oversight and support of the healthcare needs of the people we support in a residential setting. This will also include additional support to the residential staff in the programs we serve. The Licensed Practical Nurse will work in conjunction with the Director of Nursing, Registered Nurse and the Residential Administrative team to ensure the delivery of the highest quality of integrated health and continuity of care to the people residing in our residences.
