LPN Private Duty Nurse Position Available In Sullivan, New York

LPN Private Duty Nurse Community Home Health Care Liberty, NY 12754 Community Home Health Care (formerly Accu Care Home Health) has been specializing in providing Private Duty Nursing for both Pediatric and Adult clients for over 35 years in the Capital District and surrounding Counties. We are the most experienced and knowledgeable PDN Licensed Home Care Service Agency in the area. We are looking for an LPN to staff a case for an adult male in Liberty, New York. The client has a g tube, trach, is vented and is a hoyer lift. Client is authorized for 7 days a week, 8:00 am-4:00 pm or 12 hour shifts 3-4 days a week. There is flexibility in scheduling and nursing has the ability to choose their own hours/days.

Collaborate with healthcare professionals to develop and implement care plans. Maintain compliance with HIPAA regulations and uphold BLS and CPR certifications.
