SUMMARY:

As a LicensedPractical Nurse, you will provide professional patient care tolong-term and post-acute care residents. You will direct andmonitor the care provided by CNAs and consistently ensure properdocumentation and recording is taking place. Your ability to leadis crucial in both the success of the CNAs and the care of thepatients.

RESPONSIBILITIES

Responsible for timely , accuratemedication administration of assigned team and concurrent requireddocumentation. Performs treatments and documents progress of sameas per physician orders and facility protocol. Assures concise,pertinent and complete resident care documentation usingcomputerized medical record process. Assures the initiation ofappropriate care pathway tools. Responsible for receiving/givingrelevant reports, both written and verbal from/to appropriate staffas per departmental protocols. Assures proper monitoring ofresidents and relevant condition status are reported to chargenurse/supervisor and/or physicians in a timely manner. Completespre-reviews of medication administration records as per facilityprotocols. Completes required MDS documentation and all relatedobservations in a timely manner . Supports CNA team throughassisting in direct ADL care of residents.

REQUIRED QUALIFICATIONS

Graduate from an accredited School of Practical Nursing. Currentlicensure in State of New York.

PREFERRED QUALIFICATIONS

: One yearexperience preferred .

• Basic Life Support
