Schedule:
Monday-Friday 8a-5p with monthly weekend rotation What We offer:

Providesdirect patient care as defined in State Nurse Practice ActImplements current nursing practice following a comprehensiveassessment and the plan of care Provide accurate and timelydocumentation of patient services to reflect the plan of careAssess and provide patient and family/caregiver education andinformation pertinent to diagnosis and self-plan of careParticipates in coordination of hospice services, appropriatelyreporting the identified needs to the interdisciplinary group.
Examples include hospice aide, OT, PT, MSW, Dietitian or ClinicalSupervisor Provides appropriate pain/symptom management. Evaluatespatient’s response to treatments/medications
Qualifications:
Graduate of an accredited practical nurse or vocational nursingprogram with current license to practice in the state. Prior homehealth or hospice experience preferred w/ minimum of 1 yearmedical/surgical experience. Maintains a current CPR certification.
