$30 – $34 Sign on bonus: $1,200 Day shift and $1,750Night Shift Maxim Healthcare is hiring for a Licensed PracticalNurse (LPN) to work on our Transition team. Our Transition Team isa supplemental care unit of skilled nurses whose primary functionis to quickly and efficiently get patients home while primarynursing staff are actively recruited. This team is deployed afterwe receive a patient referral for home care services and thepatient is admitted to Maxim. Transition Team nurses provide carefor all authorized hours in a home during a provisional periodbetween 1-90 days. During this time, our office staff will berecruiting to identify full-time, replacement home healthcarenurses. The new caregivers will have an opportunity to train andshadow in the home with the Transition Team caregivers prior toworking independently with the patient. Once the replacement nursesare trained and have a schedule, the Transition Team are thentransitioned to another new admission.

Responsibilities:
- Utilize the nursingprocess to assess, plan, implement, and evaluate patient care.
- Oneach assignment, provides skilled nursing care/services inaccordance with prescribed orders
- Assess signs and symptomsindicating physiologic and psychosocial changes in the patient’scondition.
- Collects, analyzes, and interprets data andinformation from health care members and documents actual and/orpotential nursing diagnoses.
Qualifications:
- Currently licensedas an LPN in the state in which the LPN will practice.
- Current TBor Chest X-Ray.
- Current BLS card.
