Contract Job Duties:

Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs. Assist in the development, implementation, and evaluation of individualized care plans. Observe residents for changes in condition and report concerns to the supervising nurse or physician. Document all care provided accurately and timely, in compliance with regulatory standards. Collaborate with interdisciplinary team members to ensure comprehensive resident care. Provide education and emotional support to residents and their families. Ensure a safe, clean, and supportive environment by following infection control and safety procedures. Supervise and provide guidance to Certified Nursing Assistants (CNAs) as needed.

Job Requirements:

Active New York LPN license BLS Certification (required) Minimum 1 year of experience in

LTC/SNF Schedule:
