32 Every other weekend required. #MED1 Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients in various settings, including nursing homes and hospice care environments. The LPN will work under the supervision of registered nurses and physicians, ensuring that patients receive high-quality care tailored to their individual needs. Responsibilities Administer medications and treatments as prescribed by healthcare providers. Monitor patient vital signs and report any changes to the supervising nurse or physician. Assist with daily living activities, including bathing, dressing, and feeding patients. Perform catheter care and IV insertion as needed. Maintain accurate patient records using electronic health record systems such as eClinicalWorks and Epic. Collaborate with healthcare team members to develop and implement individualized care plans. Educate patients and their families about health management, treatment plans, and wellness practices. Ensure compliance with healthcare regulations and standards of practice. Experience Previous experience in a nursing home or hospice care setting is preferred. Familiarity with ICD-9 and ICD-10 coding is a plus. Experience in a teaching hospital environment is advantageous but not required. Strong communication skills and the ability to work effectively in a team-oriented environment are essential.
