Afew details about the role:

• Participate with members of theinterdisciplinary team to review, plan, coordinate and evaluateresident’s care.
• Document the resident condition and nursingneeds accurately and in a timely manner. Reports pertinentobservations and reactions regarding residents in a timely manner
• Develop, direct, and monitor nursing assistant assignments andadjust based on census and level of care required.
• Executetreatments as necessary and document reactions to medications andtreatments.
• Generate, prepare, administrate, and chart allmedications.
• Initiate physician orders
• Facilitate communicationwith families regarding change in medications and/or changes in theresident.
• Establish protocol to ensure care plans are continuallyreviewed for updated and accurate data that represents theindividuality of the resident.
• Coordinate admissions, discharges,and transfers in order to deliver quality customer service. Andhere’s what you need to apply:
• Licensed Practical Nurse graduatefrom an accredited nursing program.
• Certifications and LicensedPractical Nurse license and other licensure required by stateregulations.
